     9am-12pm & 1.30pm – 4.30pm.

3.  Post- using the right hand side of your prescription, tick the medication you

4.  At Reception - a prescription request box is available at reception.  You may
use either the right hand side of your prescription or write a short note
detailing your request clearly.  Please include your name, date of birth and a
telephone number in case we need to contact you.

Repeat prescriptions should be ordered 24hrs in advance & can
be collected next day at the practice. You can also inform
reception staff if you would like your prescription collected by one of the local

Patients on repeat prescriptions should have their medication reviewed at regular intervals.  If the practice has not been in contact with you concerning this, please make an appointment with your GP.
